2008 Eastern District Fall Championships

        PLEASE CHECK YOUR TEAM'S SCHEDULE PRIOR TO LEAVING FOR THE TOURNAMENT FOR POSSIBLE CHANGES.

Team members do not need to attend check-in; one adult with all required documentation is required to be present at Check-in.

This is a championship tournament; teams are permitted to play only players rostered to the team prior to the roster freeze deadline of November 1.

           TEAM REQUIREMENTS

All teams are required to bring the following items to Team Check-in:

ROSTER: A current Fall 2008 Official Roster signed by a responsible officer of the Member Association that includes all players rostered to the team as of November 1.

PLAYER ID CARDS: Current Affinity Fall 2008 Player ID Cards. Each player must have a properly prepared ID Card containing the required information (name, date of birth, ID Number, Team Code and Team Name); be signed by the player and registrar; be laminated; and contain a recent photo of the individual named on the card. PLEASE place the ID Cards in alphabetical order for a faster check-in process.

PENALTY POINT REPORT: Teams must have a copy of their Penalty Point Report which contains the date and type of penalty point (yellow or red cards) were received for each player, coach, assistant coach or trainer on the team. The report must also indicate the date that any required suspension was served and signed by the designated league adminitrator.

ADULT PARTICIPATION PASS: All adults must have a current 2009/2009 (YELLOW) Adult Participation Pass confirming they are properly registered, have cleared the required background check, have the required coaching license or are within their first year of coaching.  Four adults per team will be allowed on the team sideline during games. ALL adults on the sideline must have their Adult Participation Pass visible at all times.

Shirt or alternate jersey - In the event there is a color conflict, the home team will be asked to change to an alternate color.  If your team does not have an alternate jersey, please be sure that all players have a t-shirt of an alternate color.
